Purpose of the Assignment

                  The purpose of this assignment is to allow the student the opportunity to complete an in-depth analysis of a health issue in order to serve as an expert witness advocating for community health nursing and consumers.  An issue involves controversy and therefore may include positive and negative aspects. Students are expected to present a sound logical argument regarding the issue assigned

Student Approach to Assignment

          In completing this assignment last year, 2017, I was able to research my own position on the Medicare for All bill. It helped me to learn about the health care system in the United States and I found out some surprising population statistics. For instance, there are about 28 million uninsured citizens in the U.S. This assignment aided me to become an advocate through writing a letter to a state senator regarding the health care bill.

Reason for Inclusion of this Assignment in the Portfolio 

This project has interesting information on different populations and how they have access to health care. These populations are populations insured through work, Medicaid, Medicare, veterans, Native Americans, and those who are uninsured. I have also attached the appeal that I have sent to Senator Warner.


Critical Thinking

  • Uses decision-making skills in making clinical or professional judgments

                Based on the significant positive impact that the Medicare for All bill provides, I was able to use decision-making skills in making professional judgments. For instance, if the Medicare program has provided promising healthcare assistance for the past 51 years, Medicare for All will be able to provide coverage for many more people. This is my presumption that I based on the judgments I have made. 


Nursing Practice 

  •  Applies appropriate knowledge of major health problems to guide nursing practice

Researching the Medicare for All bill enhanced my knowledge about costs, legislation, and different populations as they relate to the healthcare system. It was an eye-opener. For instance, there are 28 million uninsured people in the U.S. and the U.S. spends $9,237 on health care costs per capita. The knowledge I gained about the uninsured US population through my research aided me to apply and navigate my notion on the Medicare for All bill.


Communication

  • Expresses oneself and communicates effectively with diverse groups using a variety of media

This was my first time writing to a senator. At first, it made me feel a bit nervous about expressing myself on a political matter but I felt much more confident that I got to communicate and advocate. I communicated through the National League for Nursing website to show expertise as part of a diverse group of nurses. I sent an email addressing my petition on the Medicare for All bill


  • Accesses and utilizes data and information from a wide range of sources to enhance patient and professional communication

I utilized the data and information from a wide range of sources. For instance, I retrieved data from National Public Radio, The New York Times, and the Journal of Health and Politics. The data and information that I gathered supported and enhanced my professional communication on the Medicare for All bill. 


Teaching

  • Uses informatics to enhance one’s own knowledge base to support teaching

The whole researching process for this assignment helped me develop my research skills. Thus, the use of informatics enhanced my own knowledge. Especially, when I was working on this paper it was my first semester in the RN-BSN program. For instance, I learned that the American Nurses Association supports the single-payer health system and believes that health care is a basic human right. This knowledge that I gained through research regarding the Medicare for All bill will be useful for me educate anyone who is interested in being knowledgeable about the bill.


Research

  • Shares research findings with colleagues

The research findings in this project give insight to any reader as to why the Medicare for All bill is the best solution for medical disparities and a better cost-reduction system. For example, under the bill, the Federal government will be a single-payer system resulting in most of the health insurance plans being eliminated. Having a single-payer system will eliminate additional administrative costs that occur with multiple insurance companies and, it will be easier for the consumers as well as providers.  I shared my research findings with my colleagues. They were not only impressed but many of them agreed on the Medicare for All bill.


Leadership

  • Assumes a leadership role within one’s scope of practice as a designer, manager, and coordinator of health care to meet the needs of populations

. This assignment aided me to assume and understand the role of a nurse advocate and a coordinator. It helped me promote self-advocacy in order to meet the needs of thepopulations effected by health insurance disparities. Thus, I was able to write a letter to a state senator. 


Culture

  • Considers the impact of research outcomes and, the effects of health and social policies, on persons from diverse backgrounds

This assignment made me research diverse populations including populations insured through work, Medicaid, Medicare, veterans, Native Americans, and those who are uninsured. When I consider the impact of my research outcome which is having a single payer system explains how the Medicare for All bill will positively affect these populations.

CULTURAL EXPLORATION

NURS 401 – Professional Development

Purpose of Assignment

            The purpose of this assignment is to develop in the learner an awareness of the health beliefs / practices of a culture different from their own


Student Approach to Assignment

In completing this assignment last year, 2017, I had to choose a culture other than mine to fulfill the assignment guidelines. I chose the Hispanic population in the U.S. due to the fact that they are one of the largest minority groups. Through the assignment, I learned many different aspects of Hispanic culture in relation to healthcare practices.

Reason for Inclusion of this Assignment in the Portfolio 

This project is very informative regarding healthcare cultural practices among Hispanics. There are different segments that explain cultural beliefs related to communication, how health is maintained, health belief practices related to pregnancy, birthing process and childrearing, health belief practices specific to end of life & death rituals, physiological variations within the culture that affect treatment or response to treatment, and the role of nursing within the culture.


   Critical Thinking

  • Uses decision-making skills in making clinical or professional judgments

After completing this assignment, I found a great deal of interesting, culturally-sensitive information regarding Hispanic/Latino culture. I learned that I have to make decision-making skills that would benefit the patient, so that I could make appropriate professional judgments. For instance, if there is a language barrier when communicating with Hispanics, I should look for an interpreter rather than choosing a family member. It will help the nurse to elicit accurate information rather than relying on the information of the family members.           

             

Nursing Practice 

  •  Implements nursing care practices as appropriate to provide holistic health care to diverse populations across the lifespan

This assignment aided me in understanding different healthcare practices among Hispanics. For instance, I need to be aware of the end-of-life practices related to Hispanic culture, especially Catholic last rites. Understanding the culture-specified practices will enable me to implement nursing care practices for holistic care to this diverse population across the lifespan.

As I have mentioned earlier, if there is a language barrier when communicating with Hispanics, I should look for an interpreter rather than choosing a family member. It will help me to adapt to communication methods to patients with special needs.

When I take care of Hispanic patients, I apply research-based knowledge that I gained in this assignment. For instance, eye contact is less common during communication. Understanding culturally sensitive practice helps me to not be judgmental.

This assignment provided me with an awareness of Hispanic population-specific health care practices. I learned to demonstrate sensitivity to beliefs influencing end of life practices. For instance, before the burial, the family will hold a wake. Family members consider this event as a social event rather than a traditional ritual.

Now I am more knowledgeable about a culture that is different from mine. I will be a better healthcare provider, since I gained abundant knowledge through completion of this assignment. I will be to able integrate knowledge of cultural diversity in performing nursing interventions. For instance, Hispanics tend to have misconceptions regarding birth control practices. They believe that those who use birth control would be unable to conceive another child.  Some of the birth controlling practices include drinking herbal teas after a sexual encounter (Nursing 322, 2010).

NURS 402 – Nurse as Educator

Purpose of Assignment

            Purpose: The purpose of the assignment is to provide the student the opportunity to summarize in detail the development, implementation, and evaluation of an instructional intervention for a specific patient or professional group.


Student Approach to Assignment

          This particular assignment assisted me in expressing my vocal abilities for nursing education. My oral presentation was on pressure injury prevention. The intended audience was the nursing staff at my current work. I chose pressure injury prevention for the educational project since it is one of the major concerns. I incorporated three educational domains (Cognitive, Psychomotor, and Affective) for the presentation. 

Reason for Inclusion of this Assignment in the Portfolio 

            This project includes pressure ulcers prevention education methods utilizing three educational domains. It also shows my personal abilities to use media such as YouTube for educational purposes. The presentation shows the development of the teaching project.

I used Bloom’s Taxonomy of Learning Domain model for this assignment. It enables the different learners to grasp the knowledge according to their preferred domain. These domains are cognitive, psychomotor and affective.

Although I have personal experience and knowledge about pressure ulcers, I interviewed the nurse educator and the wound care doctor prior to making this project. Their support enabled me to attain the goal of application of appropriate knowledge.

This teaching project presentation was an opportunity to express myself through oral presentation. It required many skills. I had to understand the different levels of education and experience levels among the nursing staff, and I spent time practicing and preparing for the presentation.

This video was a fragment of the whole teaching project that I did for the NURS 402 class. In order to make the oral presentation I had to do, I did one on one interviews with the nursing staff, director of nursing, wound doctor, and nurse educator. I was able to share all these research findings with colleagues through the oral presentation.

I organized the development strategies to promote a healthy community at my work. The preventing pressure injuries project was aimed towards the long-term care residents in my workplace. The objectives were planned for promoting pressure ulcer prevention via educating the nursing staff.

NURS 417 – Nursing Informatics

Purpose of Assignment

The purpose of this assignment is to expand informatics skills by evaluating 2 healthcare education websites using the PEMAT tool developed by AHRQ.

Student Approach to Assignment

       This assignment helped me to evaluate and analyze healthcare education websites using the Patient Education Material Assessment Tool (PEMAT) tool. I did not know there was a tool to evaluate healthcare education websites. I learned new evaluation strategies through completion of this assignment.

Reason for Inclusion of this Assignment in the Portfolio 

This assignment shows my knowledge of applying the PEMAT tool to evaluate healthcare education websites. Also, this assignment is resourceful for a person who does not know about the PEMAT tool. It will be a great learning opportunity.


Nursing Practice 

  •  Establishes and/or utilizes outcome measures to evaluate the effectiveness of care

The PEMAT tool is a great way to measure the effectiveness of healthcare education websites. For instance, the website may explain hypoglycemia care. It will be difficult to understand the meaning of hypoglycemia for a person who is not familiar with medical terminology. However, this project explains how to choose the right healthcare educational website.


Communication

This assignment expanded my nursing informatics skills. As I have mentioned earlier, I had no idea about the PEMAT tool. This assignment is promoting skills in using nursing informatics. This will be useful for me to teach my patients and fellow nursing staff so that it will assist them to look for the right and safe healthcare educational websites. It will support safe nursing practice.


Teaching

The project improved my use of nursing informatics knowledge by learning how to compare two healthcare websites. People use nursing informatics more than ever before. At times, they self-diagnose health conditions by reading online websites. The knowledge that I gained from the PEMAT tool through this assignment will be useful for educating others so that the patients can choose the right website.

I was able to share my PEMAT comparison findings with my fellow colleagues. Many did not know about the PEMAT tool but some RNs were familiar with it. It was nice, I was able to share my knowledge with my staff.

PEMAT assignment managed to promote healthy communities. It provides healthcare website evaluating skills. These websites have a lot of information on health promotion. I work in a skilled nursing long-term care facility. Pressure injuries are one of the major concerns, and I wanted to target my teaching project towards the nursing staff. It was a successful project.

This project represents my leadership skill. It also shows how I have collaborated with other nursing staff. Moreover, the projects demonstrate how I have applied learning domains to my project.

I had to utilize outcome measures to evaluate the effectiveness of the teaching segments, since it is correlated with the effectiveness of care. For instance, I educated the CNA staff on how to provide perineal care and apply Triad Hydrophilic barrier cream to those who were at risk for pressure injuries in the sacral area. Nurses learned how to apply skin prep to high-risk bony prominences. I evaluated psychomotor learning objectives through returned demonstration.

This teaching project presentation was an opportunity to express myself effectively with RNs, LPNs and CNAs. It required many skills. I had to understand the different levels of education and experience among the nursing staff, and I spent time practicing and preparing for the presentation.

Purpose of Assignment

            Purpose: The purpose of this paper is to guide you through a process of self-reflection and self-evaluation to discover the scope of your personal and professional development while enrolled in the RN>BSN nursing program at Old Dominion University.

Student Approach to Assignment

This assignment helped me to reflect on my personal and professional development during the RN-BSN program. It was a great feeling to reflect on my accomplishments. It took me a few weeks to reflect and produce this paper.

This project demonstrates the achievement of the core competencies through the attainment of specific elements within them. This paper will be a showcase of my accomplishments during the RN-BSN program.

I used the Roy Adaptation Model to guide my professional practice. If there is any concern, my first step is to find out the focal stimuli and contextual stimuli. For instance, I work with many patients with status post hip surgeries. During the admission, I gather data pertaining to the Roy adaptation modes. This is done through a comprehensive health assessment. I identify focal stimuli and contextual stimuli pertaining to physiologic mode. Status post hip-surgery serves as the focal stimuli, whereas history of falling serves as the contextual stimuli.

I was able to evaluate nursing care outcomes through the acquisition of data and the questioning of inconsistencies. I.e., at my work, we used to have bed alarms and chair alarms as fall preventive measures. When our nursing team evaluated the nursing care outcomes for bed alarms and chair alarms, it was evident that bed alarms and chair alarms did not provide enough support to prevent falls. Our nursing team decided to discontinue bed and chair alarms since they did not prevent falls.

Our nursing team at work revises actions and goals based on evidence rather than conjecture. As a nursing team, we had to revise actions and goals of fall prevention based on evidence. Having a bed/chair alarm free facility was not an easy change in the beginning, but the evidence-based literature and evaluation of nursing care outcomes assisted our nursing team to make the decision of having a bed/chair alarm free facility.

I apply appropriate knowledge of major health problems to guide nursing practice. Since I work in a long-term care skilled nursing facility, I take care of patients who have major health problems such as diabetes, hypertension, Parkinson’s, Alzheimer’s, and CVA. For instance, when I take care of a patient with diabetes, I make sure the diabetic patients follow the diabetic diet, check blood sugar levels, assess diabetic patients’ feet for any sores, and monitor for signs and symptoms of hyper or hypo glycemia.

I perform therapeutic interventions that incorporate principles of quality management and proper safety techniques. I learned to implement nursing interventions for groups of people. I learned it from community health nursing class (NURS 492). I had to do an assignment on a community health project. When I brainstormed, I realized that I am already promoting community health at the skilled nursing facility through managing chronic health conditions among the patient population. Moreover, I perform therapeutic intervention, which includes that when rehabilitation patients walk in the hallway, I ensure that they are wearing non-skid socks, and that they wear gait belts. I also make sure to provide supervision. This practice supports maintaining proper safety techniques during ambulation.

I demonstrate an awareness of complementary modalities and their usefulness in promoting health. I.e., in the NURS 363 class, I worked on a research proposal on, “Will the Use of a Relaxation Technique, Such as Music Therapy, Decrease the Usage of Opioids During the Management of Patients Age 40-45, Post Knee Athroplasty?” Through the assignment, I learned the complementary modalities and their usefulness in promoting health. I was able to demonstrate my knowledge at work by incorporating music therapy and aroma therapy for the patients who are in pain. There are times I have encouraged them to meditate, and I noticed a difference. Residents reported low pain levels.

I use therapeutic communication within the nurse-patient relationship. In order to promote therapeutic communication, I incorporate my communication skills such active listening, seeking clarification, observing non-verbal language, and reflecting. I adapt communication methods to patients with special needs. At my job, I work with patients with dementia. When I communicate with them, I talk about one thing at a time so that it will be easier for them to follow. Also, I listen to them without interrupting. I refined these skills during the RN-BSN program.

I demonstrate skills in using technology, informatics, and communication devices that support safe nursing practice. For instance, I get to demonstrate skills in using technology at work by the use of Matrix clinical charting for documentations, utilizing email to communicate with fellow nursing staff and other interdisciplinary staff members, and using the fax machine for communication with homecare department and the pharmacy. 


Teaching

 I provide teaching to patients and/or professionals about health care procedures and technologies in preparation for and following nursing interventions. For instance, there are times I work as a preceptor for the new nurses who join our nursing team. My teaching includes nursing interventions and nursing technologies -- i.e., teaching how to use the bladder scanner. I also teach nursing assistants, residents and residents’ families. For instance, I did a teaching project for my nurse educator class (NURS 402). The teaching project was focused on nursing assistants and fellow nurses.


Research

Although I had basic knowledge about research skills, this program helped me to refine the knowledge I already had. I took Nursing Research class (NURS 363) last spring. In this class, I learned the differences between quantitative versus qualitative data collection. Before the class I had an idea about data collection. Moreover, I learned to differentiate between descriptive nursing literature and published reports of nursing research. For instance, I learned to distinguish primary data and secondary data.

In the Nursing Research class, I engaged to produce a research proposal on “Will the Use of a Relaxation Technique, Such as Music Therapy, Decrease the Usage of Opioids During the Management of Patients Age 40-45, Post Knee Athroplasty?”. This was as application of research-based knowledge from the arts (Music) to complement nursing practice. I learned about effects of music therapy, aroma therapy, and medication for pain management. I apply research-based knowledge at my work, especially for patients who need pain management.

I assume a leadership role within one's scope of practice as a designer, manager, and coordinator of health care to meet the needs of populations. For instance, my current role as a charge nurse assisted me to become a leader/designer at my work. I was able to design and implement my teaching project on pressure injury prevention. It is a very common health concern at the facility.

Working as charge nurse enables me to delegate and supervise the nursing care given by nursing assistants. For instance, I ensure that they provide ADL care, monitor intake and output, and monitor the weights of the patients

I understand the effect of legal and regulatory processes on nursing practice and health care delivery.  For instance, at work it is mandatory to take the class on abuse every year. It teaches types of abuse and legal and regulatory process on nursing practice and health care delivery. Also, I learn any new state regulations every year due to annual state inspections.

I demonstrate accountability for my own professional practice. For instance, I supervise the nursing care given by others while retaining accountability for the quality of care provided. Working as charge nurse enables me to delegate and supervise the nursing care given by nursing assistants. Also, I work as a preceptor for the new nurses who join our nursing team.

I have advocated for professional standards of practice using organizational and political processes. For instance, to prevent CAUTIs, it is the standard of care for the nursing assistant to provide catheter care to patients q shift. If  a nursing assistant violates this standard, I advocate by addressing the issue.

In Community Health Class (NURS492) I learned about global environmental factors that may affect delivery of health care services. For instance, climate change and exposure to hazardous substances in the air, water, soil and food can contribute to health concerns. At my work, we ensure that we dispose hazardous waste according to facility protocol. Moreover, our facility does not use bleach for cleaning purposes since it is against the Occupational Safety and Health Administration (OSHA).

NURS 403 – Transition to BSN practice


Purpose of Assignment

The purpose of this paper is to define a clinical problem, review the literature for appropriate research supported nursing interventions, critique current practice and provide a plan for changing or adapting clinical practice.


Student Approach to Assignment  

This assignment opened my awareness to a significant clinical problem at my work: medication errors. I discussed the current situation and came up with nursing interventions that can help the clinical issue. 

Reason for Inclusion of this Assignment in the Portfolio 

This assignment is a proof addressing a clinical issue at my work. It has nursing interventions and a plan for adapting clinical practice. Also, I have elaborated my personal experience with the clinical issue as well.

Critical Thinking

  •  Uses decision-making skills in making clinical or professional judgments

In this assignment, I have listed how I used decision-making skills on a clinical situation and how I made the best clinical judgment. For instance, I was faced with an embarrassing and potential medication error moment when medication cabinets were not locked during the state inspection. This event happened in 2017 when the state was inspecting our facility. Both state and federal inspector were following me during medication administration. I was the nurse who is in charge of the 7-11 shift in my unit. Each resident has a medicine cabinet located in each room. When I went to a resident’s room for medication administration, I found that the medicine cabinet was not locked. My first reaction was to stop the medication pass and assess other rooms in the unit in order to ensure that other medication cabinets were locked. My critical thinking skill was very effective. I was able to find a few other cabinets that were not locked. I locked each unlocked cabinet prior to the medication administration.  


  • Engages in creative problem solving

This assignment made me engage in creative problem solving. For instance, labeling medications, increasing reporting, learning from medication incidents, getting rid of expired and discontinued medication on a regular basis, and minimizing error prone abbreviations are some of the solutions for the clinical issue. 


Nursing Practice 


  • Performs therapeutic interventions that incorporate principles of quality management and proper safety techniques

Through this assignment, I was able to perform therapeutic interventions that incorporate principles of quality management and safety techniques. For instance, the facility should have a scheduled day to go over each medication cabinet and get rid of the medications that are expired and discontinued. These medications can lead to potential medication errors as well. Implementing a schedule can promote better patient outcomes and avoid potential medication errors.


Teaching

  • Provides teaching to patients and/or professionals about health care procedures and technologies in preparation for and following nursing or medical interventions

In order to implement the plan of change, first step was to provide teaching to professionals about the importance of uninterrupted medication administration.


Research

  • Shares research findings with colleagues

This project is aimed towards the nursing staff at my work. My plan is to share the research findings with colleagues. For instance, it is my plan that my colleagues will adapt a “no interruption” zone so that it will reduce medication errors.



Leadership

  •  Assumes a leadership role within one’s scope of practice as a designer, manager, and coordinator of health care to meet the needs of populations

I assumed a leadership role within one’s scope of practice as a coordinator of health care to meet the needs of populations. This project required coordinating skills. It requires coordinating with the nursing staff, non-nursing staff, and nursing administration. I had to request their coordination to make this project successful because coworkers interrupt amid medication administration and the nursing administration expect the nurses to answer the nurses’ phone at all times.


Professionalism

  • Understands the effect of legal and regulatory processes on nursing practice and health care delivery

I understand the effect of legal and regulatory processes on medication administration. For instance, the usual protocol for the medication errors encompasses filing a medication error incident report. This incident report has few requirements. First, the nurse who finds the medication error must file the incident report and explain the incident descriptively. Then, she has to notify the primary care provider, and the resident’s Power of Attorney (POA). Finally, the nursing administration will do any further investigation.

COMMUNITY HEALTH PAPER

NURS 492 – Community Health Nursing

Purpose of Assignment

            The purpose of this assignment is to involve students in the comprehensive community needs assessment. Students will develop/utilize tools to identify perceived factors (both negative and positive) that impinge on the population’s health. This information will then be used to develop and implement strategies for health promotion. 

Topics have been identified from Healthy People 2020 Objectives. Presentations should be developed around one objective from the Healthy People 2020 focus areas


Student Approach to Assignment

          This assignment made me develop strategies for health promotion.  I had to choose a topic from Healthy People 2020 focus areas. I chose fall prevention.

This project demonstrates development and planning strategies for fall prevention at the facility where I work.

Critical Thinking

  • Evaluates nursing care outcomes through the acquisition of data and the questioning of inconsistencies

I was able to evaluate nursing care outcomes through the acquisition of data and the questioning of inconsistencies. I.e., at my work, we used to have bed and chair alarms as fall preventive measures. When our nursing team evaluated the nursing care outcomes for bed and chair alarms, it was evident that bed and chair alarms did not provide enough support to prevent falls. Our nursing team decided to discontinue bed and chair alarms since they did not prevent falls.


Research

  • Evaluates research that focuses on the efficacy and effectiveness of nursing interventions

There were alternative nursing interventions that I evaluated for efficacy and effectiveness. When our nursing team evaluated the nursing care outcomes for bed and chair alarms, it was evident that bed and chair alarms did not provide enough support to prevent falls. Our nursing team decided to discontinue bed and chair alarms since they did not prevent falls. Moreover, according to evidence-based practice, alternative interventions are medication reviews, assessments of visual impairment, multi-factorial risk assessment, physiological/cognitive behavioral group, flooring, monitoring, and aromatherapy. These interventions require more time. In particular, flooring can be very expensive and time-consuming (Booth, Logan, Harwood, & Hood, 2015). 


 Teaching

  • Provides relevant and sensitive health education information and counseling to patients, and families

This community project was an opportunity to provide relevant and sensitive health education information and counseling to patients, and families. For instance, one of the objectives was to ensure residents with a high risk of falling and their families were knowledgeable about fall intervention methods at the facility. I accomplished this through one on one counseling.


Leadership

  • Initiates community partnerships to establish health promotion goals and implements strategies to meet those goals

I initiated this community project so that it would establish a health promotion goal and objectives to meet that goal. My goal was to improve clinical care to prevent falls. Some of the objectives were to educate the resident families on fall prevention methods relevant to the facility and to ensure the residents were following appropriate muscle strengthening programs.
